According to the Regulation of the Minister of Health of February 17, 2003 on monitoring the safety of medicinal products, the entities responsible within Eubioco Sp. z o.o. have created a system for monitoring adverse drug reactions. We kindly ask you to report and submit any information regarding the occurrence or suspected occurrence of an adverse reaction associated with the use of a medicinal product manufactured by Laboratorium Galenowe Olsztyn Sp. z o.o. The reporting forms provided in the next section can be filled out by physicians, pharmacists, pharmacy technicians, nurses, midwives, patients or their caregivers, paramedics, or laboratory diagnosticians. The information provided in the form is confidential.

Note! Required information includes: patient details (initials, gender, or age), drug details, reporter details (name, surname, phone number), and a description of the adverse reaction.
